The CHCSS00140 Case Management Skill Set provides community service professionals with the skills and abilities to manage complex cases. You will develop skills in planning and implementing case management processes, facilitating and reviewing and evaluating case plans, and conducting holistic assessments of clients. The skill set is tailored to the needs of professionals working in community services, aged care, health care, and disability support, who wish to augment their skills and knowledge in case management. Upon successful completion of the course, you will be able to coordinate services and manage client outcomes personally or as a collaborative activity, as well as develop and enhance their own professional ability to manage complex, and at times diverse community service cases.
