The CHCSS00075 Chronic Disease Self-Management Skill Set provides care and support workers with the skills to enable clients to self-manage their chronic illness. With This program you will be able to assess clients' chronic disease needs, support self-management strategies, and promote clients' independence, wellness and improved health outcomes. This skill set is valuable for individuals working in aged care, community services, healthcare and disability support who are seeking to improve and enhance their ability to support clients in self-managing their chronic disease. The course prepares you to offer individuals practical information, motivate them to pursue healthier lifestyle options, and provide client-centred care that encourages clients to assume personal responsibility for their chronic disease.

This fee can be paid by an employer, a family member, a friend, or another organisation, but it cannot be paid or waived. The fee amount represents the total costs to the student for the qualification, including materials costs.
